With seven aged care villages across NSW, six within the Sydney Metro area and the seventh located in regional NSW.

We are seeking a Full-time Transition Nurse for our Bexley Village on an 11 month contract starting first week in November 2019.

You are a member of the Village Leadership Team and you are a passionate advocate for person centred care. In consultation with the Care Manager, you are responsible for the pre-admission assessment of needs and desires of prospective residents and their families. You oversee and coordinate the ACFI and admission assessment process within your village. Your real value is in your unique ability to achieve efficient and accurate clinical care documentation with skillful transition services, understanding and meeting the individual needs of residents. 

Required:

  • Registered Nurse Div 1 (registered with AHPRA) with a current practice certificate
  • Minimum of 4 years nursing practice in Aged Care
  • Demonstrated knowledge and application of the ACFI process
  • Commitment to the provision of person-centred care
  • Willingness to learn and to be coached on-the-job
  • An understanding of aged care legislation, regulations, and standards
  • Highly developed interpersonal skills
  • Strong abilities in assessment and documentation
  • Intermediate computer literacy

 

Desirable:

  • Ability to speak Italian or other languages
